I found out, like a credit report, you are entitled to receive a free report every 12 months. The company that handles DAC reports is USIS. their official web address is www.usis.com once there click on the consumers link at the bottom of the page and it will instruct you how to get your report. you can call them at 800.322.9651 they are open mon - fri 8am to 5pm in central time! Good luck!
